Sensory work plan

Describe one environmental barrier, propose a practical change and agree how to review it.

Every field is optional. This is a planning and communication aid, not a diagnostic test or treatment.

Entries stay in this open page: this tool sends and stores none of them. Closing or refreshing can erase them. Copy what you want to keep into a private place. On shared devices, clear entries and take any printed copy with you.

Sensory work plan

Write a few words, or print this blank sheet and use a pen. Leave any field blank.

Illustrative example: overlapping conversations

Task / barrier: checking invoices while nearby calls overlap. Impact: I lose the line I am checking and repeat entries. Request: use an available quiet desk for invoice work; if unavailable, agree a quieter work period. Constraint: I must remain reachable through the agreed urgent channel. Review: after the next invoice batch, discuss whether entries were easier to follow and whether urgent contact worked. Outcome: record what actually happened, rather than predicting success.

Illustrative example: glare

Task / barrier: reading screen text at a desk facing afternoon glare. Impact: I repeatedly move away from the desk. Request: try an agreed alternative desk position; if unavailable, discuss blinds or a suitable screen arrangement. Constraint: preserve clear walkways and lighting required for the job. Review: the next comparable afternoon shift. Check whether reading became easier and whether a new problem appeared.

Both examples are invented. They do not prescribe equipment or establish a legal entitlement.

A short request

“When I do [task] in [setting], [barrier] leads to [work impact]. Could we try [change]? If that cannot work, could we discuss [alternative]? Let us agree when and how to review it.”

Informal workflow changes and formal accommodation processes can involve different steps. Ask an appropriate workplace or local adviser about documentation and rights. Leicestershire Partnership NHS Trust discusses individualized workplace adjustments in a UK context; its guidance does not decide an individual case elsewhere.
