Task restart card

Save your place in work you have already started, then choose one action when you return.

Every field is optional. This is a planning and communication aid, not a diagnostic test or treatment.

Entries stay in this open page: this tool sends and stores none of them. Closing or refreshing can erase them. Copy what you want to keep into a private place. On shared devices, clear entries and take any printed copy with you.

Task restart card

Write a few words, or print this blank sheet and use a pen. Leave any field blank.

Two illustrative examples

These are ordinary invented scenarios, not case histories.

A half-written email

Place: reply to the delivery enquiry, draft folder. Done: answered the first question. Next: check the delivery date in the order record, then add it. Question: does the date include installation? Before repeating: check sent mail and the thread before sending.

An unfinished household form

Place: saved form, address section. Done: name and contact details. Next: find the postcode and fill the next blank field. Question: which address applies? Before repeating: check for a submission confirmation.

No note after an unexpected interruption?

  1. Find the last saved or completed part.
  2. Check consequential actions before repeating them.
  3. Write just the next visible action and begin there, if it is still the priority.

Replace “continue form” with “open the saved form at address; check the postcode.” If five fields are too much, use one line: Stopped at __; next __; check __. For driving, machinery, cooking and care, follow the relevant safety procedure before making a note.

This is an optional planning aid. NIMH describes organization and daily-task difficulties in ADHD; it does not establish that this card treats them.
